Mural Vegetation In Infective Endocarditis- Is It A Predictor For Embolism?

Abstract
Background: Mural vegetation is a condition where the vegetation is attached to the endocardium of the cardiac chambers. Identifying mural vegetations not only helps in the diagnosis of infective endocarditis (IE), but also predicts the risk of developing embolism. The aim of this study is to find out the occurrence of systemic embolism in patients with mural endocarditis.
Method: A retrospective study was done in patients with a diagnosis of IE between 2012 and 2018 at a tertiary care hospital. A total of 58 patients with definite diagnosis of infective endocarditis were identified, of which 8 patients were excluded based on the exclusion criteria.
Results: Totally 50 patients were identified with IE, of which 7 patients had mural vegetation. Blood cultures were positive in 71.4% and 88.3% of IE patients with and without mural vegetation respectively. Among 43 patients without mural vegetation, 88.3% patients had vegetation attached to the valves in echocardiogram. Size of the vegetations were 136 and 14.6 2.9mm, in patients with and without mural vegetations respectively (p>0.005). All the patients with mural vegetation and 9.3% without mural vegetation had cerebral embolic events(p<0.005).
Conclusion: Though mural vegetation is not common, if present, it helps in diagnosis and it may also predict the propensity for an embolism. Results indicate that in patients with mural endocarditis, the propensity for systemic embolism is significantly more than in patients without mural vegetation. Systematic search for mural vegetations in patients with infective endocarditis helps in risk stratification for the propensity for systemic embolism.
