Assessment of right ventricular strain in patients with reduced LV ejection fraction and LBBB by 2D speckle tracking echocardiography

Abstract
Heart failure is a complex clinical syndrome that can result from any structural disorders that impairs the ability of the ventricle to fill with or eject blood (1). The prognostic role of RV structure and function in various cardiovascular entities, like heart failure, has been proven (2).
Right ventricular shape, geometry, and position in the chest are responsible for the challenging, and demanding echocardiographic assessment of its size and function(3).
RV longitudinal strain (RVLS) derived from speckle tracking echocardiography (STE) has recently emerged as a novel indicator with less angular dependence and can identify subtle myocardial involvement (4).
Left bundle-branch block (LBBB) is a conductive disorder that causes electric activation dyssynchrony between the right ventricle (RV) and the left ventricle (LV), and between the septum and the left ventricle free wall (5). Such abnormal electric conduction and activation asynchrony may lead to early active leftward contraction of the septum within the isovolumic contraction time, followed by lengthening, while the late-activated lateral wall starts to contract (6).Abnormal motion of the ventricular septum has been described by echocardiography and is referred to as a septal flash (SF) (7).
This study aimed to investigate the effect of LBBB on RV function by 2D speckle tracking imaging in patients with HFrEF
