PREVALENCE OF PSYCHIATRIC COMORBIDITY IN CHRONIC OBSTRUCTIVE PULMONARY DISEASE PATIENTS AND THEIR PERCEIVED SOCIAL SUPPORT- A CROSS SECTIONAL STUDY IN A TERTIARY CARE HOSPITAL IN TIRUPATI

Abstract
BACKGROUND
Chronic obstructive pulmonary disease (COPD) is a long standing lung disease which has substantial extrapulmonary effects which might affect the extent of symptoms in every single patient.. COPD has a significant impact on psychological well-being of people affected. Hence there is a need to assess the prevalence of psychiatric comorbidity in COPD patients.
METHODS
100 patients with COPD were selected for the study. Severity of chronic obstructive pulmonary disease was assessed using GlobalInitiative for Chronic Obstructive Lung Disease (GOLD) criteria. Sociodemographic profile was collected by means of a semi strucured proforma. Socioeconomic status was assessed using kuppuswamy scale. Perceived social support was evaluated using Social SupportQuestionnaire 6 (SSQ6). General health questionnaire 28 item version was used to screen for the possible presence of psychological distress. Paired t test was used for Quantitative data and chi square test was performed for Qualitative data.
RESULTS
72.83% had psychiatric comorbidity. Of these, 49.38% had depressive disorders, 16.04% had anxiety disorders, 7.40% had Alcohol dependence syndrome. The impact of age and Socioeconomic status on psychiatric comorbidity was significant with p values of 0.0016 and 0 respectively.
CONCLUSIONS
Depressive disorders are the most common psychiatric comorbidity associated with COPD followed by anxiety disorders. Severe depression was the most common depressive disorder and panic disorder was the most common anxiety disorder. COPD patients with comorbid psychiatric disorders had lower perceived social support compared to COPD patients without comorbid psychiatric disorders.
