    Abstract

Warfarin is a widely used oral anticoagulant that can lead to bleeding complications, including intracranial hemorrhage (ICH). This case report describes a 68-year-old male with a history of atrial fibrillation and ischemic stroke who developed warfarin-induced ICH. The patient was started on warfarin therapy and monitored with INR measurements. On the eighth day of therapy, the patient presented with confusion and slurred speech and was found to have a large ICH on CT scan. The INR level was 5.5, well above the therapeutic range. Warfarin therapy was immediately stopped, and the patient was given Vitamin K and fresh frozen plasma to reverse the anticoagulant effect. Despite aggressive medical management, the patient's neurological status continued to deteriorate, and he eventually became comatose. The patient's family ultimately decided to withdraw care. Warfarin-induced ICH is a rare but potentially fatal complication of anticoagulant therapy, and its management requires prompt diagnosis and treatment.
