    Comparative Study of Cytomorphological Robinson’s grading for Breast Carcinoma with Modified Bloom-Richardson Histopathological Grading

    Abstract

    Invasive breast carcinoma is most common carcinoma in women. Fine-needle aspiration cytology is the important part of triple assessment plays a major role in primary diagnosis of breast carcinoma. Cytological grading can provide prognostic information and aid in planning the management options. Aim: To grade breast tumor on cytology according to Robinson cytological grading and to correlate with histopathological grading. Material and Methods: A prospective study was carried out in Department of pathology, GRMC Gwalior. Fifty fine-needle aspiration smears of breast carcinoma were graded and correlated with Histopathological findings. Robinson grading was done on cytological smears and Modified Bloom-Richardson grading was done on histological sections. Results: In present study, Cytologically most common tumor was grade II (48%) followed by grade I (34%) and grade III (18%). Histologically, most common tumor was grade II (52%) and grade I (24%) and grade III (24%). Concordance rate was high in grade II (87.5%) with absolute concordance being 78% (p< 0.005). Most of the tumors were in the postmenopausal and premenopausal women with mean age of presentation was 51 years. Conclusion: Robinson cytological grading in fine needle aspiration smears in breast carcinoma is the simpler and feasible grading method and can be used as a prognostic factor for better management of patients. This provides relevant information regarding aggressiveness of tumor and to plan treatment preoperatively.
