    Screening of pap smear according to Bethesda 2001

    Abstract

    the primary goal of cervical cytological screening is to identify women whom further evaluation with colposcopy is required to detect the presence of the cancer precursor. The development of cervical cancer proceeds in a particular fashion over a long period which gives ample opportunity for intervention at a precancerous stage. Material and method: This is a hospital based prospective study carried out over the period of one year(1st march 2008 to 1st march 2009) .All patients coming in gynaecology OPD were included in this study, complaining of discharge per vaginum, PV bleeding, something coming out of vagina(scopv), acyclic bleeding, itching ,post coital bleeding, postmenopausal bleeding and elderly patients were included as apart of screening program. Results: We studied pap smears of 1002 patients with detailed clinical history and clinical examination over the period of 1 year. The pap smears were reported according to the Bethesda classification 2001. Out of 1002 cases, 157(15.66%) were NILM,336 (33.53%) were NILM with specific infection,458(45.70%) were NILM with nonspecific inflammation,31(3.09%) were epithelial cell abnormality, 20(1.99%)were unsatisfactory for evaluation. Summary and conclusion: we found that maximum incidence of infection, inflammation was seen in reproductive and postmenopausal age group of poor hygiene(83.08%) and illiteracy(87.99%). Incidence of NILM(Negative for intraepithelial lesion or malignancy) as 94.91% which includes cases of nonspecific infection and specific inflammation. Incidence of cancer was 1.79%.
