    To compare criteria based discharge (CBD) scoring system to the existing time based discharge (TBD) criteria in decreasing length of stay (LOS) in patient undergoing unilateral knee arthroscopy under general anaesthesia with ultrasound guided adductor canal block versus general anaesthesia with intravenous analgesic methods.

    Abstract

    The aim of the article is to compare criteria based discharge (CBD) scoring system to the existing time based discharge (TBD) criteria in decreasing length of stay (LOS) in patient undergoing unilateral knee arthroscopy under general anaesthesia with ultrasound guided adductor canal block versus general anaesthesia with intravenous analgesic methods. Result: The modified aldrete score was calculated in each patients in both groups at the interval of 10minutes in PACU till they achieved the score of 9 or above. It was observed that mean CBD time in morphine group was 25.50±10.12 minutes and mean CBD time in block group was 14.00±6.72 minutes which was found to be clinically significant. Various non clinical factors responsible for delay in the discharge of patients from PACU to the wards were studied. It was observed that unavailability of housekeeping staff to transfer patients from PACU (40% in morphine group and 30%in block group). Conclusion: This significantly helps in better utilization of PACU resources for more sick patients by reducing the work load of PACU. As PACU is higher dependency and costly unit because of the level of monitoring and care required in it, by decreasing the length of stay (LOS) of patients in PACU. To decrease the overall length of stay (LOS) in PACU, we should also take care of other non clinical factors which accounts for delay in discharge.
