Outcomes in Early Vs Delayed Laparoscopic Cholecystectomy for Acute Cholecystitis - an Observational Study
Abstract
The study aims to evaluate and compare the clinical outcomes of early and delayed laparoscopic cholecystectomy in patients with acute cholecystitis. Material and Methods: Thirty patients, with acute cholecystitis, who had undergone laparoscopic cholecystectomy were studied. The patients were deemed to have undergone early laparoscopic cholecystectomy if they had their surgery within 72 hours of the initial presentation of the symptoms. All those patients, who underwent the surgery after 4 weeks, were considered to be in the delayed laparoscopic cholecystectomy group. Results: The early and delayed groups had 19 and 11 patients respectively. The overall occurrence of intraoperative complications was found to be similar in both groups. It was 84.2% in the early laparoscopic cholecystectomy group, and in comparison, it was 90.9% in the delayed group. Major bleeding in the early group was 21.1% and that of the delayed group was 18.2%. The bile and gallstone spillage, in our study, among the early and delayed groups were 47.4% and 72.7% respectively. In our study, the drain requirement in the delayed group was 63.6%, while it was 52.6%, in the early group. The duration of surgery on average was 140.5 minutes in the early group when compared to 95.5 minutes in the delayed group. Conclusion: There were no statistically significant differences in the outcomes of early and delayed laparoscopic cholecystectomy in patients with acute cholecystitis regarding complications. Abstract
It has been observed that the changes in the systemic arterial circulation, portal hypertension, commencement of vasoconstrictors and inhibition of vasodilatory factors acting on the renal circulation were the factors which led to development of renal impairment in CLD and cirrhosis. Material and Methods: This study was done in Department of Biochemistry in Prasad Institute of Medical sciences, Banthra, Lucknow, U.P. Total 43 cases were included in this study. The duration of study was over a period of two year. Results: Out of the 43 patients of cirrhosis, the cause of liver disease was attributed to alcoholism in 21 patients. 6 patients were found to be positive for Hepatitis B surface antigen. Out of the 43 patients, renal function was assessed by serum creatinine, creatinine clearance from timed urine collection [(UxV)/P] and creatinine clearance by Cockcroft Gault formula (CGF). Conclusion: It can be concluded that estimation of Serum creatinine alone has not been found to be a reliable marker to assess renal dysfunction in CLD.

Effect of Pulmonary Rehabilitation Programme on BODE Index in Comparison to Pharmacological Therapy alone
Abstract
Pulmonary rehabilitation (PR) is a key component for treatment of Chronic Obstructive Pulmonary Disease (COPD). The risk of morbidity/mortality in patients with COPD is often graded with the use of a single physiological variable, the forced expiratory volume in first second (FEV1). The BODE index, a simple multidimensional grading system is a better tool than FEV1 alone to evaluate the patients with COPD. Presumably, a higher BODE score correlates with an increased risk of death.4 Objective: The objective was to assess the effect of PR on BODE index and compare it with the change in BODE index of patients on conventional pharmacological therapy alone. Methodology: In this prospective study, 60 confirmed cases of COPD were divided in two groups of 30 patients each and both groups were put on adequate pharmacological treatment as per GOLD guidelines. Patients of study group were enrolled in PR program in addition to the pharmacological treatment. BODE index was calculated for both the groups at diagnosis, after 7 days and after 6 weeks. Results: In both groups, significant improvement was observed in the BODE index from the time of diagnosis till first week and time of diagnosis till the sixth week. However, improvement in BODE index from first week till the sixth week was statistically significant only in study group. Conclusion: PR shows significant improvement in BODE index over longer duration than pharmacological treatment alone, hence potentially reduces the risk of death in COPD

The two cardinal factors in the maintenance of normal physiology of the paranasal sinuses and their mucous membranes are drainage and ventilation. Normal drainage of the paranasal sinuses depends on effective mucociliary clearance; this is dependant, among other things, on the condition of the sinus ostia.1 OBJECTIVES: To study the various sinonasal anatomical variations in the study population. MATERIAL & METHODS: Study Design: Prospective Hospital based observational study. Study area: Department of E.N.T, Narayana Medical College and Hospital, Nellore, Andhra Pradesh. Study Period: August 2016 to July 2017. Study population: All the patients attending the E.N.T. outpatient department, who had chronic sinusitis for more three months duration not responding to the medical line of treatment and who were willing to undergo Functional Endoscopic Sinus Surgery. Sample size: study consisted a total of 50 cases. Sampling method: Simple random method. Results: The uncinate was typical in 29 (58%), medialized in 22 (44%), anteriorly turned in 1 (2%), hypertrophied in 6 (12%) and pneumatized in 2 (4%). The superior attachment of the uncinate process was as follows: middle turbinate in 21 (42%), lamina papyracea in 18 (36%) and skull base in 11 (22 %). Ethmoidal bulla: The bulla was typical in31 (62 %), large in 11 (22%) and hypoplastic in 8 (16 %). CONCLUSION: In view of the presence of these significant variations, we reemphasize the need for proper preoperative assessment in every patient in order to accomplish a safe and effective endoscopic sinus surgery.

Study population: The study population included all the cases with HF admitted at a tertiary care center Sample size: 100 Results: majority of cases were found in above 70 years group e.g.70 followed by 51-60 years age group 22 cases, 61-70,41-50,31-40,21-30 and 18-30 found 17,15,14,5 and 1 case respectively. majority of study participants were Males contributing 60 (60%) and Females 40 (40%). majority of cases had a history of Hypertension 86, followed by Smoking were present in 40 cases, Alcohol history reported by 37 cases, 23 cases were found with DM, Rheumatic heart disease in 17 cases and metabolic syndrome found in 15 cases. Majority of cases complained Dyspnoea on exertion e.g. 89 followed by Orthopnoea in 73 cases, Pedal oedema in 67, Chest pain 56, Palpitation 49,cough 40,syncope 40 and other compained reported by 56 cases. majority of cases discharged 81, DAMA 12 cases and 7 cases death during treatment Conclusions: Heart failure was seen commonly in age group of above 70 years. most common risk factor was HTN. Abstract
Objective: The aim of this study is to assess the serum uric acid levels in patients with T2DM attending a tertiary care hospital. Methods: This cross-sectional study was conducted in a tertiary care hospital in Mahabubabad,Telangana.. A total of 100 patients diagnosed with T2DM were included in the study. The serum uric acid levels were measured using standard laboratory methods. Data on demographic characteristics, medical history, and medications were collected through a structured questionnaire. The data were analyzed using descriptive statistics and inferential statistics. Results: The mean age of the study participants was 57.3 years, and 55% were male. The mean serum uric acid level was 6.7 mg/dL. 41% of the participants had hyperuricemia (serum uric acid level ≥ 7.0 mg/dL). There was a significant positive correlation between serum uric acid levels and age, body mass index (BMI), fasting blood glucose (FBG), hemoglobin A1c (HbA1c), and serum creatinine levels (p < 0.05). No significant association was found between serum uric acid levels and gender or duration of diabetes. Conclusion: The study found a high prevalence of hyperuricemia in patients with T2DM attending a tertiary care hospital. Serum uric acid levels were positively associated with age, BMI, FBG, HbA1c, and serum creatinine levels. These findings suggest that routine monitoring of serum uric acid levels may be beneficial in identifying those at risk of developing cardiovascular and renal complications in patients with T2DM. Further studies with larger sample sizes are needed to confirm these findings and establish the clinical significance of these associations

THE EFFECTS OF HIGH-INTENSITY INTERVAL TRAINING ON CARDIO VASCULAR FUNCTION IN SEDENTARY INDIVIDUALS
Abstract
Aim: To investigate the effects of HIIT on cardiovascular function in sedentary individuals. Methods: Fifty sedentary individuals (25 males and 25 females) were recruited and randomly assigned into either an experimental (HIIT) or a control group. The experimental group underwent a 12-week HIIT program, while the control group maintained their sedentary lifestyle. Cardiovascular function was assessed by measuring resting heart rate (RHR), blood pressure (BP), and maximal oxygen consumption (VO2max) at baseline and after 12 weeks.Results: At baseline, there were no significant differences between the HIIT and control groups in RHR, BP, or VO2max. After 12 weeks, the HIIT group had a significant decrease in RHR (from 75.2 ± 8.3 bpm to 68.3 ± 7.5 bpm, p < 0.001) and BP (systolic BP from 124.7 ± 11.5 mmHg to 118.3 ± 9.9 mmHg, p = 0.001; diastolic BP from 78.6 ± 8.5 mmHg to 72.9 ± 6.8 mmHg, p < 0.001), and a significant increase in VO2max (from 31.6 ± 4.1 ml/kg/min to 36.8 ± 4.3 ml/kg/min, p < 0.001) compared to the control group. In contrast, there were no significant changes in RHR, BP, or VO2max in the control group.Conclusion: The results suggest that HIIT may be an effective intervention to improve cardiovascular function in sedentary individuals. These findings have important implications for developing interventions to prevent or treat cardiovascular diseases in sedentary populations. Future research should investigate the long-term effects of HIIT on cardiovascular function and other health outcomes in sedentary individuals.

Structured 10 item questionnaire presented to them in their vernacular language before and after counseling about prevailing myths and beliefs and same questionnaire done after 7 days in follow up. Pretest, Posttest and follow-up answers collected systematically in form of knowledge, attitude and practice and analyzed. RESULTS:- Mean knowledge score was 0.4 ± 0.491 before counseling and it was 0.94 ± 0.231 after counseling and 0.9 ± 0.302 at first follow up at 7 days. Mean attitude score was 0.33 ± 0.47 before counseling and 0.82 ± 0.385 after counseling and 0.8 ± 0.401 at first follow up. Mean practice score was 0.3 ± 0.458 before counseling and 0.99 ± 0.104 after counseling and 0.7 ± 0.458 at first follow up. A significant association was found in all KAP scores before and after counseling with P value <0.05. CONCLUSION:- In this study we observed pre discharge counseling about myths and social beliefs have a significant impact on their knowledge, attitude and practice. It is recommended in all NICU settings and it may have a large impact on community level too.

Assessment of the Standardized Reference Range for HOMA – IR among Apparently Normal Population of Ottappalam
Abstract
Aim: To determine the standardized reference range for HOMA – IR among apparently normal population of Ottappalam. Methodology: 100 apparently normal subjects were selected and subjected to blood pressure measurement, Weight and height, BMI, Triglyceride, HDL and LDL cholesterol and HOMA – IR was calculated. Results: Age group 20-25 years had 5%, 26-30 years had 80%, 31-35 years had 9%, 36-40 years had 3% and >40 years had 3% patients. The mean age was 28.78 years. Most of the subjects taken for the study were females – 62%. Most of the subjects were in the 18.5 – 22.9 kg/m2 group (94%). Minimum value of HOMA – IR was 0.74 and maximum value was 5.38. Mean HOMA – IR for the apparently normal population of Ottappalam was calculated as 2.65. Range for HOMA – IR according to 95% confidence intervals was 2.46 to 2.84. HOMA – IR had a positive correlation with age, which means as age advances the insulin resistance also rises, p value for this observation being 0.023. HOMA – IR had a positive correlation with BMI, p value being <0.001. HOMA – IR had a positive correlation with both systolic and diastolic BP. Fasting plasma insulin levels had positive correlation with BMI, systolic and diastolic blood pressures.

Managing Peritonitis Secondary to Hollow Viscus Perforation: An Analysis of Perioperative Complications at a Tertiary Care Centre
Abstract
Peritonitis caused by hollow viscus perforation is a life-threatening condition that requires emergency surgical care and is associated with high rates of morbidity and mortality. Therefore, the operating surgeon faces a challenge to provide appropriate management that can lead to a better outcome. As a result, a thorough evaluation is necessary. Method From August 2020 to July 2021, a study was conducted at IMS BHU. The study involved analysing data from 192 cases of peritonitis caused by hollow viscus perforation. The study focused on identifying the causes of the condition, surgical interventions, and peri-operative complications. Appropriate statistical analyses were performed to draw conclusions. Result Of the 192 cases analysed, males accounted for154 while females were 38 with an average age of 45 years. The primary cause of peritonitis was gastric ulcer (GU) perforation, followed by idiopathic, infective, malignancy, appendicular perforation, and trauma.

Significance of serum ferritin, interleukin 6 and erythrocyte sedimentation rate as a marker of COVID 19 disease Severity.
Abstract
Inflammatory responses play a critical role in the COVID-19 disease. These Markers have been reported to be significantly associated with the high risks of developing severe COVID- 19. Our study highlights the role of interleukin 6 (IL6), erythrocyte sedimentation rate (ESR) and serum ferritin in predicting the severity COVID-19. Objectives To investigate and correlate inflammatory markers Erythrocyte sedimentation rate (ESR), Interleukin 6 and Serum Ferritin with the severity of Covid-19. Key words Covid 19, Interlukin6, Serum ferritin,Erythocyte sedmentation rate,Disease severity. Results Out of 307 patients, we found all the three inflammatory markers (IL6, ESR and serum ferritin) correlate with disease severity the mean ESR in mild, moderate and severe COVID 19 disease was 35,42 and 55.5 respectively. The mean IL6 in mild, moderate and severe COVID 19 disease was 14,33 and 61 respectively and the mean serum ferritin in mild, moderate and severe COVID 19 disease was 253,373 and 801 respectively. Conclusions These inflammatory parameters could be used in for prompt identification of severe covid-19 disease patients and subsequently triaging them for initiation of treatment protocols
